Guidance notes for completing this form and how SBHA HomeChoice will process your application.
General
You must answer ALL questions.
Any questions that are marked as Mandatory, in red, must be filled in before you can move on to the next question.
If you have to stop before completing the form, click on the Save button at the top of the application form. You will be given a provisional application number. To re-open your form, choose Display Saved Application from the side menu and input your provisional application number.
Mutual Exchanges
If you are an SBHA tenant, you have the right to apply to exchange your home with other tenants. This is called a mutual exchange.
In order to exchange, you must obtain permission in writing from SBHA. If you would like to join the Mutual Exchange Scheme, please contact any SBHA office and a form will be sent to you. If you want a mutual exchange, do not complete this form.

Preparatory Questions to Building the Form
You must tell us how many other people need to be housed, in addition to yourself. For example, if you are single, select Myself Only or, if your partner and 2 children are to be included, select Myself and 3 others.
If anyone included on your application has lived at a different address within the last 5 years, you should mark the Yes box and you will be asked about previous addresses later in the form.
Your Details
Tell us whether or not you are Existing Tenant of SBHA. In order to answer Yes, you must be the tenant not simply a member of the family.
You must enter your title, first name(s) and last name, as well as your date of birth and whether you are male or female.
You must enter your National Insurance Number
